Please note, a referral from your specialist is almost always required for an intake

For your first appointment we will ask you over the phone for your date of birth, address, and if applicable your BSN number, so that we can immediately check your insurance for you.

By car you can drive up to the barrier at the main entrance of the building. The barrier will open automatically. Drive up to building D and park at one of the parking spots with a green Manometric sign (parking is free).
